General Rules for Tygerberg Hospital Patients

Please bring the following with you to the hospital:

• Hospital card if you have one
• Road to health card for a child (baby card)
• Identity document or some form of identification like a residence permit or passport
• Proof of income
• Pensioner's card if you have one
• Medical aid card if you have one, and your authorisation number for specialised procedures. Please call your medical aid in this regard.
• Referral letter from a doctor. This is essential when you are attending a central hospital
• The medicine you are taking currently.
• Proof of address, e.g. a municipal bill indicating your home address
• Please notify the hospital of any change in your address
• If possible, call the clinic or ward to confirm your appointment
