This form need to be completed to make a nomination for the Arts and Culture, Language, Library, Museum, Heritage and Archive Awards 2009. The Western Cape Department of Cultural Affairs and Sport seeks to recognise outstanding achievements and contributions in the areas of Arts and Culture, Language, Museums, Heritage, Libraries and Archive Services in the Western Cape. These awards are open to any individuals, organisation or group whose efforts have made a difference to the communities within the Western Cape. The nominees should be 18 years and older, and should be South African citizens.
